Individual support and advocacy for adults experiencing problems with mental illness, substance use, or co-occurring disorders.

Services are provided in the participant’s home or other community settings to access community resources, to develop skills needed to reside in the community, and to prevent exacerbation of illness or admission to higher levels of care.
