SOAR stands for SSI/SSDI Outreach Access and Recovery.

It is technical assistance and support with disability benefit and other mainstream resource applications.

This program is for individuals who are experiencing homelessness and have a serious mental illness.

If you or someone you know are experiencing 3 out of the 5 conditions, please contact our SOAR Case Managers:

• Are you homeless or at risk of homelessness?

• Do you have a mental health diagnosis?

• Are you currently unemployed?

• Do you have a history of hospitalization and/or medical care?

• Do you still need to start your application?
