P.I.E.R. - Assertive Community Treatment

People living with a severe, persistent mental illness (SPMI) find treatment, rehabilitation, and support in the Partnership In Empowerment and Recovery program.

P.I.E.R. consumers:
• Receive a full range of intensive services including therapy, medication management, and more in their home or other community setting
• Work toward a life not dominated by their mental health disorder
• May experience a reduced need for hospitalizations and/or fewer issues with the justice system
• Are empowered by living as independently as possible in the community

P.I.E.R. is a collaboration of CenterPointe and Lutheran Family Services.

Thank you for your interest in referring to the PIER program! Whether you are the individual interested in services, a provider who has someone in mind, or a guardian seeking help we are happy to talk with you about your referral to determine if this is the right program! 

When submitting a referral please fill out the fillable referral form with as much information as possible as well as have the individual and/or guardian fill out and sign releases of information to any agencies or providers where the individual has received psychiatric, medication, inpatient, counseling or crisis services within the last 2-5 years. 

You can request AND send those completed forms to referrals@pierlincoln.org Once we have received the completed referral forms we will begin the records review process. Once this step is complete we will reach out to the referral source to discuss the next steps.
